I authorize a charge of $19.95 for single phase service or $29.95 for three-phase service to be placed on my monthly Gulf Power bill and I agree to the commercial surge protection program terms and conditions. The commercial surge protection program device will be installed in the next few weeks by our contractor, Itron. Your power will be turned off for up to 15 minutes during the installation. Commercial surge protection is a program offered by Gulf Power that is not regulated by the Florida Public Service Commission. Your choice of whether to participate in the program will not affect the price, availability or terms of service from Gulf Power.
